| Instructions For Coaches:
Have your player's Parents/Guardians fill out the form and
return the completed form to you.
You must keep the completed forms with you at all GBSL games
and practices. They are for use in an emergency if the injured player's
Parent/Guardian is not present, or cannot be reached in a reasonable and prudent
timeframe relative to the seriousness of the situation/injury.
