Referee Evaluation Form
Form Directions:
- As a Coach in GBSL, we value your
opinion as to the performance of the Referees that have refereed your
games. Please take some time and complete this form after each
game. Do not use this form just for complaints or problems; we would
like to hear about the good jobs that are done also. The New York West
Referee Association Board and/or the State Referee Administration will
address any comments about improper behavior by a Referee.
- Complete the form by filling in
the blanks, selecting the appropriate choice and adding any comments.
- When you have completed the form,
simply click the "Submit" button and your responses will be
automatically sent to GBSL.
